Scleral Lens Application and Removal Guide

We now have an Application and Removal Guide to provide to patients. This guide is a supplement to the Healthy Lens Habits Guide. Our goal is to provide the simplest yet most effective education for patients and to provide you with tools to support your success.

Healthy Lens Habits Guide

These miniature guides are shipped with every lens order and provide basic instructions for lens care. The guide includes write-in areas where you can indicate your recommended cleaning and disinfecting solutions for each patient.

Application and Removal Guide

Our newest guide includes lid spread photos and instructions with photos on how to apply lenses with a plunger and without, and how to remove lenses with a plunger and without. We have professionally printed versions of the Application and Removal Guide available for you to provide to your patients. This single-page version folds into a 5.5″ x 8.5″ guide for easy storage. In the meantime, download the 8.5″ x 11″ printable version for office use here.

Manny’s Tip from the Lab: Lens Wettability

One of the most important lens parameters is the material’s oxygen permeability or Dk value. Did you know that higher DK materials have a lower wettability? Patients with severe ocular surface disease may benefit from a lower Dk material and thus higher wettability. See below for BostonSight SCLERAL material options and their accompanying Dk value.

Remember that Hydra-PEG significantly improves lens wettability. Hydra-PEG coating can be added for a one-time additional fee of $25 per lens.

Ensuring Positive (Scleral) Impressions

Scleral lens impingement is caused when the lens fits too tightly and the haptic or landing zone digs into the conjunctiva. Long-term impingement can cause conjunctival hypertrophy (impression ring) and lead to increased discomfort and lens awareness for the patient. Thus, it is important to address impingement during the fitting process. Here are some tips to relieve impingement that can be reported to our lens consultants:

  • Inferior Impingement: Rule out if it is true impingement from the lens haptic being too tight vs. pseudo impingement from the lens haptic being too loose. When the latter happens, the pseudo impingement is due to the lens decentering and “falling” into the conjunctiva in the inferior quadrant. Perform a push-up test and evaluate the haptic alignment with the lens centered on the eye and re-assess the inferior haptic. If edge lift becomes apparent then the haptic needs to be tightened to better align the haptic and center the lens – this will get rid of the pseudo impingement. If during the push-up test the lens does not move because it’s too tight, then the haptic is too tight and needs to be loosened or flattened.
  • If there is a circumferential impression ring, you can loosen/flatten the haptics in each meridian. However, if upon lens removal there is, in addition, significant rebound hyperemia, refit the patient by increasing the lens diameter by 1.0mm to move the landing of the lens outside of the impression ring. A larger lens provides a wider landing area to distribute the weight of the scleral lens more evenly and minimize compression.  

In some cases, even if the lens fit is optimized the patient may still report limited wear time due to a dull ache during wear, transient blurred vision on lens removal, or rebound redness after lens removal. Scleral lens suction should be ruled out. Increasing lens diameter, and adding two (2) SmartChannels® can be added on the horizontal meridian to reduce lens suction and improve your patient’s comfort and overall “impression” of scleral lenses.

FitAssist® Vouchers for BostonSight SCLERAL Patients

What’s a FitAssist voucher?

A FitAssist Voucher is redeemable for a free BostonSight SCLERAL fitting for one patient in financial need. A voucher covers the cost of the fit charge (one or both eyes) but does not include non-refundable shipping charges, Hydra-PEG coating, and SmartSight HOA.

How do I earn a FitAssist voucher?

A voucher is issued to your practice for every 12 patients you fit and complete payment for. Vouchers are redeemed per patient and can be applied for one or both eyes. The more lenses you order, the more vouchers you are eligible for!

Do FitAssist vouchers expire?

Yes, effective June 1, 2020, FitAssist Vouchers expire 24 months from issue date.

How do I access my FitAssist vouchers?

It’s easy! Follow these steps to access your vouchers in FitConnect® or download this one-page document.

  1. Login to your FitConnect account and go to the Admin menu. Select FitAssist Vouchers.
  2. Click View on a voucher that has the status of Issued and select the patient and practitioner. 
  3. Print the voucher and have the patient attest to financial need.
  4. Scan the signed voucher and upload it to FitConnect. Click Upload on the voucher screen and browse for the scanned document on your computer.
  5. BostonSight will apply a price adjustment for the patient’s Fit order(s)!
